We are undergoing a transformation in municipal healthcare - Do you want to be part of it? Currently, the municipality's healthcare is undergoing a transformation that is part of the national reform aimed at creating a more "Good and Close Care" model. This is an ongoing change that affects how municipalities plan, organize, and deliver their assignments. The transformation aims to make healthcare more person-centered, coordinated, health-promoting, and preventive. In Sala, we have good conditions for collaboration as we have two health centers, Close Care Teams, and proximity to our collaborating partners at Sala Hospital department 1 and the Medical Clinic. Our ambition is to solve what arises together and in dialogue. We are now seeking a colleague for Jakobsbergsgården, as our regular nurse Karolina will start working nights. Jakobsbergsgården is also our on-call hub and is centrally located in Sala. Here you meet patients with multiple diagnoses/diseases simultaneously and who affect each other and require coordination. Jakobsbergsgården also has a short-term unit and two departments for elderly patients with multiple chronic conditions. Cognitive diseases are common and require a structured and person-centered approach with a focus on interaction. If you have an interest in how cognition, i.e., memory, and how the ability to perceive and understand the world is affected by dementia diseases, that is an advantage. Job duties As a nurse in special housing, you are responsible for approximately 20 patients in a patient care assignment (PAS). You plan your day based on given frameworks and resources and adapt to new conditions when the operation requires it. You make your assessments based on healthcare guidelines and regulations and plan, implement, follow up, and evaluate healthcare interventions for patients with varying disease patterns. Evening and weekend shifts are also part of your assignment: 2 weekday evenings every 8 weeks and two weekends per 16 weeks. In Sala, two nurses work on call (evening, weekend, and night) and cover the municipality's special housing, disability services housing, social psychiatry, and home care patients. Qualifications We are looking for someone who takes responsibility, has a collaboration focus, and contributes to an operation where we support each other – always with the patient's best in mind! Someone who prefers to find what is right and good rather than wrong but does not shy away if obstacles/problems arise. You are a licensed nurse and preferably with experience in municipal healthcare. It is a merit if you have specialist training in cognitive disease, diabetes, or other training in elderly care. You find it easy to collaborate with others and are accustomed to adapting to the situations that arise. You are engaged and make wise decisions, always with the patient's goals in focus. With your ability to coordinate, plan, organize, prioritize, and supervise, you contribute to safe and effective care. Beyond this, you have a driver's license category B and have good knowledge of the Swedish language in both speech and writing.
