As a care coordinator, you play an important role in this work by ensuring that patients receive the right support and interventions, from the right provider, at the right time. Work Tasks The position involves a combined assignment where you work both clinically with patients and with overall responsibility for care coordination. You have the opportunity to combine patient-centered work with creating structure, continuity, and coordination around patients who need interventions from multiple healthcare providers or professions. Clinical work includes: acute and planned reception activities, telephone advice and medical assessment, digital patient contacts. The work is conducted in close collaboration with the primary care center's various professions and with a focus on safe, secure, and nearby care. As a care coordinator, you work with: planning, coordinating, and following up care interventions based on the patient's individual needs, ensuring continuity, safety, and patient security throughout the care chain, leading and coordinating coordinated individual planning (SIP), collaborating with internal and external parties according to the applicable collaboration model (ViSam), and contributing to the development of approaches and interventions for patients with frailty and complex care needs. The role involves independent and structured work while maintaining many contact points. You become an important link between the patient, the primary care center's various professions, and other actors in healthcare and social services. Qualifications Requirements: . Licensed registered nurse or district nurse . Strong ability to make independent medical assessments . Good communication skills and the ability to communicate clearly and professionally, both in personal meetings and by telephone Meritorious: . Experience working in primary care . Experience with care coordination and coordinated care interventions . Experience collaborating with municipalities and other external healthcare and social service providers We are looking for someone who: . The area has approximately 2,400 employees within eleven different operational areas. Nearby care encompasses care that is common, frequently occurring, and geographically near patients. The area includes primary care operations with primary care centers, primary care emergency services, youth services, healthcare advice, asylum and immigrant health, general specialized care at the county's three hospitals, and geriatric clinic, as well as operations within rehabilitation and assistive devices.
