The Adult Services Unit consists of 12 social workers who work with substance abuse, addiction and relationship violence, 7 case managers, 1 housing coordinator, 2 eviction prevention specialists, 1 housing liaison officer, 2 group leaders, and 1 unit manager.
The unit is part of the adult services area together with financial assistance. Collaboration is an important part of achieving our goals and we work continuously to improve it.
We are now looking for an engaged case manager with an interest in working with people living with co-occurring disorders.

Your role
Case Management is an intervention with a coordinating function that aims for individuals with co-occurring disorders to be able to live as independently as possible. You serve as support in contact with social services and other collaboration partners such as psychiatry and addiction care.
Work tasks:
Support and follow up those who have support in the form of training or trial housing
Outreach social work for users with co-occurring disorders
Work to implement the assignment that has been developed together with social workers and users
Identify and map the needs the user has to move forward in the rehabilitation process
Work as a case manager involves much work outside the office. You work in an outreach capacity and often meet users in their home environment or at the housing and facilities where they are located.
Work in the evenings may occur.
Your competence and experience
The position requires:
Degree in social work or equivalent
Knowledge of substance abuse, psychiatry, neurodevelopmental disorders, and cognitive disabilities.
Meritorious:
Education in case management
Experience working as a case manager
Training in MI
We are looking for someone who is flexible and can adapt to changing circumstances. You stay calm in pressured situations and are persistent even when facing setbacks. As someone who takes initiative, you make things happen and with an open and collaborative approach you contribute to good dialogue. You are clear in your communication and ensure that messages get through.

Other information
To work for Stockholm's city with children, young people, the elderly, and people with disabilities including work in their homes, you must show an extract from the police register (belastningsregistret). The extract must not be older than six months from the date of issue.
