We are now seeking a case manager for elderly care in Hallstahammar Municipality – a full-time permanent position. With us, you will have the opportunity to work closely with both colleagues and residents and help ensure that people receive the right support at the right time. The case management unit currently has five case managers and one occupational therapist. We are a group that supports each other, shares knowledge, and values a good work environment. Through collaboration between case managers and the occupational therapist, we can together ensure high-quality case handling and create better conditions for residents to maintain their independence as long as possible. You will also receive support from a unit manager in your daily work. We adapt the introduction to your needs so that you get a secure start and good conditions to settle into both the work and the work group. JOB DUTIES As a case manager within elderly care, you investigate the need for services and make decisions according to the Social Services Act. The assignment includes, among other things, advisory services, planning, coordination, and monitoring of interventions. You are responsible for a geographical area where you handle cases such as home care, respite care, short-term care, safety alarms, and special housing. An important part of the work is planning related to discharge from the hospital. One case manager at the unit primarily works with discharge processes, while other case managers mainly work with cases in ordinary housing and short-term care. QUALIFICATIONS We are seeking someone with a degree in social work or another degree that the employer considers equivalent. Knowledge of social law is important for this position. It is meritorious if you have experience in public administration, particularly within healthcare and social services. Previous experience in another role within elderly care is also meritorious. It is also meritorious if you have experience with the business systems Life Care and Cosmic Link.
