We are now looking for you to work as a case manager in LSS and social psychiatry and become part of our unit within the Care Administration. You will be part of a work group with varied experience, where we actively work to develop and strengthen the LSS and social psychiatry area. About the Role As a case manager with focus on investigations in LSS and social psychiatry, you have a central and important role within the Care Administration. You work with authority and decision-making based on clear delegations and guidelines and contribute to creating vital services that make a difference in people's daily lives. A large part of the work involves planning and assessing individuals' need for support and services. You conduct home visits, which provides a varied workday where no two days are alike. The work alternates between desk time and field visits, both planned and urgent. You are also responsible for reviews and follow-ups of previous decisions. This involves handling assessments, documentation, and decisions within the social legal area. As the responsible case manager, you make decisions within your delegation, but you never stand alone – support and guidance are always readily available from both your supervisor and colleagues. The work takes place in close collaboration within the work group as well as with other professional roles and external cooperation partners. We work needs-focused according to IBIC (Individual's Needs in Focus) and are a unit that actively develops our working methods and collaborations to best meet the needs of those we serve. Qualifications We are looking for someone with higher education in social care programs, social work programs, or other education that the employer deems equivalent. The education should include social law or the disability field. You have experience with authority and decision-making in social services. It is meritorious if you have experience with case management in LSS, SoL, and/or social psychiatry, as well as knowledge or experience with the IBIC working method and a solution-focused approach. As a person, you are structured and have good ability to plan, organize, and prioritize your work. You work independently and take responsibility for moving your cases forward, while also having good collaboration skills and a professional demeanor. You are attentive and flexible in meeting with other people and find it easy to adapt your communication according to situation and recipient. You express yourself well in Swedish, both verbally and in writing, and are comfortable documenting in a legally secure manner. The position requires a Class B driver's license. Since home visits are part of the work, you need to be comfortable being in environments where pets and smoking may occur.
