The State Social Care Center "ZEMGALE" is Latvia's largest provider of state long-term social care and social rehabilitation services! We are hiring a SOCIAL REHABILITATOR at the "Jelgava" branch. We offer: social guarantees in accordance with state administration regulations; salary (gross) EUR 851 – EUR 1220 according to professional experience and competencies; salary during probation period up to EUR 870; 25% bonus to the set monthly salary for special risk; reduced working hours within normal working time (35 hours per week); work experience in state administration; additional leave for special risk (3 working days per year) and additional leave for annual evaluation (from 4 - 10 working days per year); friendly, responsive, and professional colleagues; health insurance after 3 months of work. Main job duties: assess clients' social functioning; diagnose clients' social barriers, social status, values, and determine clients' social skills level; participate in an interprofessional team during clients' social rehabilitation; participate in developing, implementing, and improving individual social rehabilitation plans for clients; improve social skills and foster new skills under the guidance of a social worker. Requirements for candidates: social rehabilitator qualification, short-cycle higher education in social rehabilitation or second-cycle higher education in social work or charitable social work, if also obtained short-cycle professional higher education and level five professional qualification in social rehabilitation, or first-cycle higher education in social work and social worker qualification, if during studies or professional development education process completed part of the study program related to social rehabilitation of at least four credit points or 160 hours; knowledge in the field of social rehabilitation, social care, health care issues, psychology and pedagogy; knowledge necessary for performing basic professional tasks at the level of understanding; work experience in the care field will be considered an advantage; valid electronic signature. We are inviting a SOCIAL CARE WORKER to work in our Jelgava branch! Requirements for candidates: Social care worker qualification, short-cycle higher education in social care or second-cycle higher education in social work or charitable social work, if also obtained short-cycle professional higher education and fifth-level professional qualification in social care or first-cycle higher education in social work or charitable social work and social worker qualification, if during studies or professional development education a part of study programs related to social care was completed covering at least four credit points or 160 hours; Desired professional experience working with persons with mental health disorders. Knowledge of regulations related to job responsibilities. Responsibility, accuracy and honesty, ability to be client-oriented. Understanding of social care worker job duties. Valid electronic signature. Main responsibilities: Provide social care services to branch clients. Help clients improve existing and acquire new social skills. Inform and educate clients on issues related to their obligations and rights, as well as the procedure for receiving social services. Ensure, at least once a year, assessment of client functional status and develop an individual social care or social rehabilitation plan in cooperation with social work specialists, according to client social care or social rehabilitation needs. Ensure transparent recording of information in client files about the progress of the social care process and make entries at least 1x per week in client files about the progress of the social rehabilitation plan. Implement client individual social care or social rehabilitation plans, modify and supplement them according to changes in client life situations. Assist clients in adapting to social and physical environment, promote integration into society. Assess client personality, self-care abilities, physical and mental health status. Monitor changes in mental state, behavior and health status of clients in care. Inform on-duty nurses, psychiatrists of changes and make entries in client file. Prepare and complete work documentation according to regulatory requirements and procedures established in the institution.
