WORK TASKS
As a care coordinator/nurse with us, you work in close collaboration with municipal services and specialist care. We have joint case reviews/meetings 2 times per week. As a care coordinator, you work in LINKEN to coordinate contacts and plan the interventions needed for the patient regardless of age, diagnosis, living arrangement, and who have a need for coordinated planning. This may mean that a care plan, SIP, team meetings, etc. are needed. There are opportunities here to develop the primary care center's working methods and routines to early identify patients with coordination needs. You are also a support for colleagues in this process.
You will also work with reception services, assessment consultations, and telephone advice. We work in a multiprofessional team and apply work with fixed physician contact/fixed care contact. There is also the possibility of working with certain specialist consultations.
QUALIFICATIONS
We are looking for you who
· are a licensed nurse or district nurse, preferably with a few years of experience in primary care.
· Have a genuine interest in primary care and patient meetings
· Work independently and structurally
· Have good collaboration skills
· Are confident in your professional role and have a professional approach
· Are engaged and want to contribute to the development of operations
· Contribute to a good work climate
Experience from primary care is a merit as well as interest in mentoring students together with your colleagues/having mentor training. We place great importance on personal suitability.
