We're now hiring an engaged planner for Home Care Youth Services in Lund Municipality – an important function where you'll have the opportunity to grow and contribute to ensuring our users receive the best possible care.
About the role and your responsibilities
You'll be employed in a regular nursing assistant position and work daytime hours 06.45–15.15 with duty every other weekend. The role includes both administrative tasks and work in daily care where you work as part of the team.
As a planner, you're responsible for planning and scheduling the approved services for our users. You have regular contact with users, family members, supervisors, healthcare staff, colleagues, and case managers. In your work, you'll use Lifecare and Mina planner. You work closely with our coordinator who is on-site Monday through Friday. The planner role is staffed by two people, and you'll take turns working in care every other weekend.
This position is a permanent nursing assistant employment. The planner assignment runs for six months at a time, with good opportunities for extension. We offer full-time work equivalent to 36 hours and 20 minutes per week.
Qualifications
Nursing assistant certification.
Experience from similar work such as home care, psychiatry, dementia care, or LSS services.
Previous experience as a coordinator/planner is preferred.
It's an advantage if you have worked with Lifecare and Mina planner.
You have strong command of Swedish, both spoken and written.
Work in Home Care Youth Services requires that you have bicycle experience and a valid driver's license.
You are responsible, engaged, flexible, and attentive – with users, family members, and colleagues. You understand how important it is that each individual is involved and has influence over their daily life.
We place great emphasis on personal suitability. It's important that you maintain calm and focus, even in situations when a lot happens at once or during rapid changes. You need to be confident in yourself and able to set boundaries when needed.
About the workplace
Home Care Youth Services has its central office at Bankgatan in central Lund. From here we visit users who live around Lund city, distributed across four areas: Linero/Tuna, Centrum/Klostergården, Värpinge, Gunnesbo/Nöbbelöv, and Fäladen.
Lund Municipality's home care is divided into several geographic areas. In each area, nursing assistants and care assistants work in teams with healthcare staff under the leadership of a unit manager. Each area has a unit administrator and coordinator/planner. We provide care, support, and home healthcare services around the clock to the municipality's users.
