You will daily document and handle any deviations. Together with colleagues you distribute work tasks and communicate with healthcare personnel such as registered nurses, rehabilitation staff, and on-call nurses. Examples of Work Tasks: Be the contact person and responsible for your clients' weekly planning and implementation plans Follow up on your clients' weekly planning and implementation plans Receive medication delegation from the registered nurse Conduct fun activities Handle documents and deviations Who Are You? We are looking for you who is a team player – a person who is responsible and enjoys working both independently and in a group. You have the ability to see the bigger picture and you are not afraid to take initiative. You are used to documentation and work with quality as your guiding principle. Experience with person-centered work is a merit, and we believe it is important that you are a relationship builder with good cooperation skills and a positive attitude. We Are Looking for You Who: Are trained as a nursing assistant or have equivalent work experience Possess good knowledge of Swedish in speech and writing It Is a Merit If You Have: Experience working in elderly care or home services Experience working with people with dementia Experience in privately-run operations A prerequisite for starting employment with us at Humana is that you as an applicant can provide a valid ID document and any documents that prove your right to work in Sweden.
