About the Operations Within Elder Care Assessment, we meet people aged 65 and older who, due to aging and its consequences, need support and assistance. We receive applications, conduct assessments, make decisions under the Social Services Act (SoL), and follow up on decided interventions. The unit consists of approximately 40 employees and is located centrally in Umeå. To strengthen quality and legal certainty in elder care, the unit has a special follow-up group. The group's task is to conduct in-depth follow-ups of approved interventions and ensure that the individual's needs are met, that interventions are carried out according to decisions made, and with high quality. The work includes both individual follow-up and review of how approved interventions are implemented in practice. Through follow-up, analysis, and dialogue, the group contributes to identifying quality shortcomings, deviations, and areas for improvement, as well as strengthening equitable, legally sound, and reliable elder care. Your Work Tasks As a social worker in the follow-up group, you will work with in-depth follow-ups of interventions approved under the Social Services Act. Your tasks include conducting in-depth follow-ups of approved interventions and monitoring that interventions are carried out according to decisions made and assignments, assessing whether the individual's needs are met and whether the interventions achieve their intended purpose, and identifying quality shortcomings, deviations, and areas for improvement. Your work also involves collaborating with individuals, relatives, service providers, colleagues, and other professionals. You work independently but also closely with colleagues and collaboration partners both within and outside your own operations. Your Role As a social worker in the follow-up group, you have an important function at the intersection of authority functions, follow-up, and quality development. The role requires you to combine professional and respectful conduct with an analytical and objective approach. You will conduct follow-ups, review documentation, analyze results, and make determinations based on legislation, decisions, and factual circumstances. You need to be confident in managing complex situations where different perspectives must be weighed and where follow-ups sometimes may lead to interventions being reconsidered or changed. You see this as a natural part of a legally sound authority assignment. About You We are looking for someone who is confident, stable, and has high integrity. You are structured and analytical while also having good interpersonal skills and the ability to build trusting relationships. You enjoy working independently, take responsibility for your assignment, and have the ability to make well-founded decisions even in complex situations. We also see that you have strong communication skills, both verbal and written, and view collaboration as a natural part of the work. Great importance will be placed on personal suitability. Qualifications We are looking for someone who has: a degree in social work with several years of experience including: current experience with authority functions under the Social Services Act (SoL) experience with assessment, evaluation, and documentation good knowledge of legally sound case management good knowledge of the Social Services Act (SoL) B-category driver's license Meritorious It is meritorious if you have: experience working in elder care or with people with disabilities experience with follow-up, quality, or development work experience with in-depth follow-ups knowledge of IBIC (Individual Needs in Focus) experience collaborating with service providers experience analyzing and assessing complex cases Welcome to apply!
