Key responsibilities:
Provide social work to adults and groups of persons, assessing their needs for social services, resources, and support systems;
Provide clients with information about opportunities to receive social services and social assistance in accordance with municipal regulations;
Assess and allocate social services, including home care, long-term and short-term institutional care services, day care center services, group apartment/boarding house services, and transportation services;
Develop social rehabilitation plans for overcoming crisis situations and provide social rehabilitation services, including to adults who have experienced or committed violence;
Organize the provision of social services according to individual needs and coordinate cooperation with involved specialists;
Conduct home visits and assess clients' daily activities and living environment, including completing assessment forms in accordance with regulatory requirements;
Prepare decisions and referrals regarding service allocation or denial, and ensure data processing in state and municipal databases;
Enter allocated services into the SOPA information system, update information, and monitor changes in the client's situation;
Create and maintain client files in electronic and/or paper form, ensuring document registration, archiving, and data protection in accordance with regulatory requirements.
Requirements for applicants:
Education: Master's degree in Social Work;
Ability to understand and apply regulatory acts in work;
Capacity to make independent decisions within your competence;
Good computer and office equipment skills;
Interpersonal skills and ability to communicate with people;
State language proficiency at the highest level;
Attention to detail, ability to acquire new information and manage large amounts of information;
Category B driver's license (desirable).
