About the operations We are now seeking a nurse with a temporary assignment as care coordinator at unit 95 Säter Hospital. County Adult Psychiatry Falun/Säter has a regional mandate in adult psychiatry and is responsible for care and treatment of patients in acute psychiatric conditions. Operations are divided between Falun and Säter. In Säter, there is the region's only psychiatric emergency clinic as well as units 90 and 95. In Falun, there is a substance abuse center, a general psychiatric inpatient unit, LARO and ECT operations. Unit 95 is a general psychiatric unit with 14 beds for patients with varying psychiatric diagnoses and extensive care needs. Patients are cared for according to HSL or LPT. Nurses, healthcare assistants and physicians work together in teams here with a focus on safe and person-centered care. The position is a permanent employment as a nurse with weekend duty. During the regular care coordinator's leave, the position includes a time-limited appointment as care coordinator with daytime work Monday to Friday. Work tasks Do you want to combine clinical work with a developmental coordination assignment? We are now seeking a nurse who, during a colleague's absence, is offered a time-limited appointment as care coordinator at our unit. Your primary employment will be as a nurse on the unit. During the appointment, you will work as a care coordinator with responsibility for supporting and developing the unit's work in nursing care, collaboration, quality and care processes. About the assignment As a care coordinator, you have a central role in ensuring well-functioning patient flows and person-centered care. Together with the unit manager and care developer, you work with coordination, quality development and implementation of new working methods. The assignment includes, among other things: Promote a person-centered approach and increase patient and family participation in care. Coordinate and follow up work related to discharge planning and collaboration with outpatient care, municipalities and other healthcare providers. Support and guide staff in nursing care, documentation and record-keeping. Educate and supervise staff regarding procedures for collaboration at discharge, care planning and personal care plans and safety plans. Participate in quality, development and improvement work and contribute to implementation of new routines and working methods. Contribute to follow-up of operational goals and care processes. Coordinate work related to user-controlled admission and contribute to ensuring that procedures for ÖPT are followed. Terms of employment Permanent employment as a nurse on the unit with weekend duty. Time-limited appointment as care coordinator during the regular employee's absence during the period 2026-10-01 to 2028-03-31, with possibility of extension. Daytime work Monday to Friday. Qualifications Requirements: Authorized nurse. Meritorious: Experience working in psychiatric care. Specialist nurse education with a focus on psychiatric care. Experience in coordination, quality or development work. Experience in collaboration between inpatient care, outpatient care and municipal operations. Experience in education, supervision or implementation of new working methods.
