Welcome to Us Health and Care Team 9 is part of the health and care function in Jönköping Municipality, which consists of 13 multiprofessional health and care teams and one home team. We have specialist focuses in psychiatry, palliative care, home rehabilitation, dementia, and functional disability care. Together we are approximately 460 dedicated employees. Our catchment area is Barnarp with countryside, Råslätt, Kettilstorp, Tokarp, Ljungarum, and Gräshagen. Here we are responsible for health and care services in four elderly care facilities: Havsörnens äldreboende, Junegårdens äldreboende, Kärrhökens äldreboende, and Ädelstenens äldreboende, as well as group home Kärrhöksgatan. The work team is based at the team office in Råslätt and consists of occupational therapists, physical therapists/physiotherapists, nurses/specialist nurses, assistant nurses, and a unit administrator. Your New Job As a nurse with us, you are responsible for both planned and acute health and care interventions in individuals' homes, regardless of housing arrangement. You meet patients of different ages and have close cooperation with both primary care and inpatient care to ensure safe and coherent care. Our work is based on person-centered care, where the patient's participation, self-determination, and sense of security are in focus. As a nurse, you make independent medical assessments and plan and implement treatments and care interventions based on the patient's needs. The role also includes guidance and training of care personnel, patients, and relatives, as well as development and quality improvement work. This is a position for someone who appreciates independent work, multiprofessional collaboration, and the opportunity to follow patients over time. Your Competence Requirements Licensed nurse. B-class driver's license, as a car is used in the position. Good Swedish language skills, both spoken and written. Good computer skills. Good physical mobility as work takes place in home environments and you need to adapt your working methods based on available conditions. Meritorious Specialist training in, for example, care of the elderly, palliative care, or district nursing. Experience working in care and health services, for example as a nurse or assistant nurse. You have good judgment, are empathetic, and contribute through your professional approach to safe and secure care. You are flexible and solution-focused, which enables you to quickly adapt when conditions change. The role requires that you are comfortable working independently and making your own decisions, while also collaborating well with colleagues and other professionals to create the best possible care for the patient.
