Help develop the Health and Medical Care function in Sweden's ninth largest municipality! Welcome to Us Health and Medical Care Team 7 is part of the health and medical care function in Jönköping Municipality, consisting of 13 interdisciplinary health and medical care teams plus a home care team. We have specialist areas in psychiatry, palliative care, home rehabilitation, dementia, and disability services. Together, we are approximately 460 dedicated employees. Our service area covers Bankeryd with surrounding countryside, Trånghalla, Kortebo, parts of Bymarken, and Järstorp. Within the area, there are four elderly care facilities: Ceciliagården elderly care home, Ekåsen elderly care home, Ramseysgården elderly care home, and Vilhelmsrogården elderly care home. The work team operates from our office at Strandängen and consists of nurses/specialist nurses, occupational therapists, physical therapists/physiotherapists, nursing assistants, and rehabilitation assistants. Your New Job As a nurse, you are responsible for both planned and acute health and medical care services in individuals' homes. This position is focused on residential care facilities. You will meet patients of different ages and work closely with both primary care and hospital care to customize care services for each individual. Our work is based on person-centered care, where patient participation, self-determination, and individual needs are in focus. As a nurse, you make independent medical assessments and plan and implement treatments and interventions based on patient needs. The role also includes coaching and educating care staff, patients, and relatives to ensure good and safe care. You will also have the opportunity to contribute to the organization's development and quality work to meet the future of municipal health and medical care. Your Competence Requirements Licensed nurse. Class B driver's license, as a car is used in this position. Good knowledge of Swedish, both spoken and written. Good computer skills. Good physical mobility, as work is conducted in home environments and you need to adapt your work approach based on existing conditions. Meritorious Specialist training in, for example, elderly care, palliative care, psychiatry, or district nurse training. Experience working in care and support services, for example as a nurse or nursing assistant. As a person, you are empathetic, structured, and receptive. You have the ability to build trusting relationships and communicate clearly and professionally with patients, relatives, and colleagues. You collaborate well with others and contribute to a positive work environment. Through good judgment, knowledge, and diligence, you contribute to safe and secure care where the patient's needs are always the focus.
