WORK TASKS
In your role as a nurse in municipal primary care, you are responsible for health and medical care services in patients' homes. Your work includes, among other things, medical assessments, preventive measures, medication management, and providing support and guidance to care personnel. You plan and follow up on care measures based on each patient's individual needs and situation.
You collaborate closely with other professionals within and outside the municipality, for example nursing assistants, doctors and rehabilitation staff. The position suits you if you enjoy independent work while valuing collaboration and have the ability to coordinate and maintain care measures. We work from a person-centered approach and a holistic perspective, where each patient encounter is unique and meaningful.
QUALIFICATIONS
We are looking for you who:
• are a licensed nurse
• have good experience working with IT-based patient record systems
• can express yourself well in speech and writing in Swedish
• have a B driver's license
Meritorious:
• specialist training as a district nurse, in psychiatry or care of the elderly or other equivalent education
• previous experience working in care and healthcare
As a person, you are confident and professional in your professional role and have the ability to remain calm and focused even in stressful situations. You take responsibility for your work tasks and work independently, while having good collaboration skills and being attentive in communication with patients, relatives and colleagues.
