We are seeking two registered nurses who want to play a central role in our growing home care services at Bromma Primary Care Clinic.
You will work full-time with our home care assignment during daytime hours on weekdays only. This role is ideal for you if you enjoy combining patient care with coordination and team leadership, and if you appreciate close collaboration with both nursing assistants and physicians.
Experience from basic home care, primary care clinics, or ASIH is advantageous, but what matters most is that you feel confident in your role as a registered nurse and want to develop home care services together with us.
About Us
Bromma husläkarmottagning is a personal primary care clinic located in the heart of Brommaplan with a mandate from Region Stockholm.
We are a small operation with short decision-making processes, high expertise, and close collaboration between physicians, registered nurses, and nursing assistants. We have a growing home care assignment, chronic disease clinics, and active development work where employee ideas are valued.
Our home care team currently consists of two nursing assistants, one registered nurse, and one home care physician. One of our valued nurses is moving on to new challenges this autumn, which means we are now seeking their replacement. We are looking for you to take an important role in our home care team and continue developing our services together with us.
About the Position
As a registered nurse in home care, you have a key role in daily operations. You work both independently and as part of a team to ensure safe, secure, and person-centered care in patients' homes.
You serve as clinical support for our home care nursing assistants and work closely with our district physicians on planning, assessments, and follow-ups.
The work is varied and combines patient visits with coordination, planning, and medical assessments.
Examples of Responsibilities
- Plan and coordinate daily home care services.
- Supervise and provide clinical support to home care nursing assistants.
- Conduct home visits and perform nursing interventions.
- Participate in care planning meetings and SIP meetings.
- Collaborate with physicians, family members, case managers, and other care providers.
- Handle incoming cases via LifeCare.
- Answer our direct phone line for home care patients during part of your workday.
- Contribute to the development of procedures and quality within home care services.
Who Are You?
We are seeking a registered nurse with experience in independent clinical work.
It is advantageous if you have experience from:
- basic home care
- primary care clinics
- ASIH
- municipal home care services
