In Linköping City Syd, Bräcke diakoni has comprehensive responsibility for healthcare, rehabilitation, and care in the home. This gives us short lines of communication between professions and excellent opportunities to plan together around the patient. We are now seeking a registered nurse who wants to be part of our team. The position is full-time with daytime work on weekdays.
Your role in the team
As a nurse with us, you will follow patients over time and serve as the responsible nurse, PAS, for your own patient group within your area. You will conduct clinical assessments, plan interventions, and follow up on care as needs change. An important part of the work is also to strengthen the patient's own resources and create conditions for participation and independence. You will become part of a multidisciplinary team where nurses, physiotherapists, occupational therapists, and physicians work closely together. We have fixed physician contacts through our health centers, recurring rounds, and multidisciplinary meetings with home rehabilitation. Together we coordinate interventions, share knowledge, and support each other when patient needs change. Here you contribute with the nurse's perspective throughout the care process.
At Bräcke Home Care Services
In the nursing team, we are six nurses. Three are responsible for Berga and Ramshäll and three for Valla/T1 and Stolplyckan. Each nurse has their own area and patient list, but we plan together and support each other in prioritizing when needs change. We also work closely with home rehabilitation through shared cases and recurring team visits. We continuously develop how we organize our work, distribute responsibility, and collaborate between professions. Here there is close leadership and support from MAS regarding quality and patient safety.

Qualifications
Registered Nurse
Experience with independent clinical assessments
Familiarity with documenting in digital care information systems
You express yourself fluently in Swedish, both verbally and in writing
Driver's license (Category B)
Meritorious
Experience in home care or municipal health and care services
Experience in geriatrics or palliative care
Experience supervising care personnel
