Rehabilitation Coordinator Combined with Another Professional Role at Premicare Timrå/Söråker
In your role as a rehabilitation coordinator, you will have a central function in supporting individuals on their journey back to work. You will coordinate, guide, motivate, and ensure that healthcare, employers, and the Swedish Social Insurance Agency work toward common goals.
We are looking for you who are a licensed physical therapist or another rehabilitation professional who also wants to take on the role of rehabilitation coordinator. A hybrid role where you can contribute to our patients' path back to health and work!

As a rehabilitation coordinator you will:
* Support patients on sick leave in the rehabilitation process and facilitate their return to work.
* Conduct mapping conversations and establish rehabilitation plans in dialogue with the patient, treating physician, and other stakeholders.
* Participate in collaboration with the health center's doctors, psychosocial team, and other staff regarding complex cases.
* Coordinate contacts with external actors such as the Swedish Social Insurance Agency, the Swedish Employment Agency, and employers.
* Conduct multi-party meetings and follow up on the patient's path back to work or other activities.
Qualifications
We are looking for you who:
* Are licensed healthcare personnel or social worker
* Preferably have previous experience as a rehabilitation coordinator
* Preferably have experience in primary care.
* Are experienced in working both independently and as part of a team.
* Have good knowledge of Swedish, both spoken and written.
* Hold a class B driver's license
