About the Operation
Britsarvet-Grycksbo is a well-functioning primary care center with two clinics, one located in Falun and one in Grycksbo. We have approximately 15,000 registered patients and approximately 75 employees. We operate physician services and nurse clinics. You will join the Behavioral Health Team, which includes psychologists, care coordinators, and an addiction nurse. Our physiotherapist is part of primary care rehabilitation, which is shared across publicly-operated primary care centers in Falun. The primary care center also includes a Family Center.
Responsibilities
Provide support to patients and physicians within the rehabilitation process.
Serve as a point of contact in relevant cases for patients, rehabilitation actors, employers, Försäkringskassan, Arbetsförmedlingen, and Social Services.
Assess rehabilitation needs, develop rehabilitation plans, and conduct motivational conversations for return to work.
Contribute to the development of internal coordination and collaboration with other actors and support the operations manager's work with monitoring and analysis of the unit's sick leave data and identify the need for skills development in this area.
Participate in rehabilitation coordinator network meetings coordinated by the Region and at the local level.

Qualifications
Required:
Higher education corresponding to licensed healthcare personnel, such as nurse, physiotherapist, occupational therapist, or social worker.
Meritorious:
Work experience in rehabilitation within healthcare or occupational health services.
Training organized by the Region for Rehabilitation Coordinators or equivalent, such as higher education in rehabilitation science and/or 7.5 credits for Rehabilitation Coordinators.
As a person, you are responsible and flexible and thrive working both independently and as part of a team. You have strong collaboration skills and drive to develop the operation. Great emphasis will be placed on personal qualities. As a rehabilitation coordinator, you will be the hub connecting everything.
