The worker provides direct care services to individuals with health disabilities.
- Training in simple daily activities and supporting clients in performing them,
- Assistance with maintaining cleanliness, personal hygiene, and dressing,
- Help with handling equipment, aids, and laundry,
- Support for client independence,
- Promoting life activation, such as supporting leisure time activities,
- Support in developing basic social and community contacts for the client,
- Support in meeting the client's psychosocial needs,
- Creates individual care plans for individual clients,
- Maintains relevant documentation,
- Fulfills other instructions from their supervisor.
Responsibilities:
- For the safety of service users,
- For the individual planning process of the service delivery in the position of key worker,
- For implementing the individual plan as a key worker in cooperation with the social worker,
- For collaboration in implementing individual service plans for clients with other key workers,
- For providing care and services in accordance with applicable methodological regulations: social service provision methodology and work procedures.
