Adult Rehabilitation provides specialized rehabilitation to adults with significant and persistent functional limitations. The operations are organized into three diagnosis-focused teams: the Autism Team, the Neuro- and Brain Injury Team, and the team for people with intellectual disabilities. The Autism Team meets adults with autism without intellectual disabilities. The team includes coordinators, psychologists, occupational therapists, and special educators. Together, we create individually tailored interventions with the patient's needs, conditions, and goals at the center. Duties As a coordinator in the Autism Team, you will have a broad, independent, and varied work area. You work both individually with patients and together with colleagues in various group interventions. Your responsibilities include: providing support in contacts with authorities and other societal actors offering supportive and motivating conversations planning, developing, and leading group activities contributing psychosocial expertise in the team's collaborative work collaborating with next of kin, care providers, authorities, and other actors adapting communication and interventions based on the patient's individual needs and conditions. Patient contacts are conducted both during physical visits and through digital healthcare meetings. You plan your work independently and in close collaboration with the other professions in the team. Qualifications We are looking for someone with a degree in social work. It is a merit if you: are a licensed health and medical care coordinator have experience working with people with autism have experience from rehabilitation or other related activities have experience collaborating with authorities and other societal actors have experience with group activities. Since the operations cover the entire Örebro region, a B driver's license is a requirement. To thrive in this role, you need to be able to work independently while also appreciating collaborative work in an interdisciplinary team. You have an easy time collaborating and building trusting relationships with patients, next of kin, colleagues, and external actors. You have a holistic perspective on the patient's life situation and can adapt both your communication and interventions according to individual needs.
