About the position As a contact nurse in hematology, you work in close collaboration with experienced colleagues in the same role, physicians, and the clinic's day care and outpatient services. Together you create safe and coherent care for patients throughout the entire care chain. In the role, you work according to the national job description for contact nurses. You participate in diagnosis discussions and multidisciplinary conferences, are responsible for establishing and revising My Care Plan, and identify needs for cancer rehabilitation. You also develop, follow up, and adapt rehabilitation plans based on each patient's individual needs. In the role, you serve as the patient's and their loved ones' fixed care contact throughout the investigation and treatment process. This means you provide information about care and treatment, offer support during crisis reactions, and ensure continuity and accessibility in contact with healthcare. In your work, you make evidence-based assessments and take relevant actions to meet the patient's needs. Over the past ten years, we have achieved significant medical advances, where contact nurses in particular have played a decisive role in and around patient care and quality of life during their contact with our clinic. The goal in the role is to ensure safe, coherent, and person-centered care for patients with hematological diseases by being a clearly designated contact person. We are looking for someone who - Is trustworthy, stable, and has self-awareness. You are attentive and agile; you listen, communicate, and resolve conflicts constructively; you are unassuming enough to step in where needed - Have the ability to put yourself in someone else's perspective or situation without taking on their emotions Qualifications Requirements: - Swedish nursing license - Several years of experience as a nurse Meriting factors: - Specialist training in oncological care - Completed contact nurse training - Previous clinical experience in hematology or oncology About the recruitment process Selection and interviews may be conducted on an ongoing basis during the application period.
