Our residences are designed to meet the specific needs of each individual. We collaborate with relevant authorities and operate throughout Sweden. Our goal is to help individuals transition from benefit dependency to self-sufficiency and independence through individually adapted housing and services. We are now looking for our next team member! Job Duties We are seeking a treatment assistant for our supported housing team. The position involves you serving as a contact person for a number of clients, whom you meet regularly. Contact includes both in-person meetings and phone calls. Your work includes supporting clients according to individual plans from the social services in collaboration with Horisontum. You will support individuals with finances related to housing, securing internships, jobs or education, and coaching them toward meaningful leisure activities that promote health and social engagement. The work may also include supporting and motivating individuals in contact with healthcare, municipal services and other agencies. Acute situations may arise during the work, which requires you to be organized, flexible and solution-focused. The goal is to help the individual create a sustainable life through housing, employment, independence and well-being. The position involves working largely independently, which requires the ability to plan, implement, complete and follow up on the individual's plan together with the individual. Horisontum is a growing company consisting of a team that requires close collaboration - therefore we see that you are a team player with strong communication skills. You will participate in weekly meetings, workplace meetings and supervision. You report to your direct manager on ongoing work. The position also includes contact with and reporting to social services and other involved authorities. We see that you have experience from a similar position and/or education focused on psychiatry, behavioral science or pedagogy. The position is based in selected parts of the country, but may also involve short travel within the region and Sweden. Therefore, a Class B driver's license is required. Qualifications Requirements Work experience from a similar position Secondary education/post-secondary education in social work and care, behavioral science, psychiatry, pedagogy, etc. Class B driver's license and access to a car as the work may involve travel within the region or country.
