We are now recruiting new staff members for the Mobile Team and Specialized Palliative Care in Skåne. The service is part of the primary care area Mobile Team and Specialized Palliative Care, which encompasses five geographic operational areas and the Palliative Development Center. The mission is to provide specialized palliative care and other advanced home healthcare services 24/7 in collaboration with other healthcare actors and municipalities. We are in an exciting development phase regarding the establishment of Mobile Team Near Care, a form of care that offers the most vulnerable patients an alternative to hospital care. We operate from eight locations in Skåne and provide care in patients' own homes, in municipal care facilities, or in a palliative inpatient ward. Enabling more patients to receive care outside the hospital is a central part of our mission and permeates our work. JOB RESPONSIBILITIES As a pharmacist with us, you will have a varied role where your expertise makes a difference throughout the entire medication chain. You will work both closely with the operations at one or more locations in Skåne and more broadly with developing, quality-assuring, and strengthening medication management. Here you will have the opportunity to influence and contribute to safe and patient-secure medication use throughout the area. Placement location to be discussed. The work involves close collaboration with colleagues, chief pharmacists, chief physicians, chief nurses, unit managers, and medication-responsible physicians on medication-related matters. You will be a support to the clinical operations on-site and as a consultant. As a pharmacist, you will also have an important role in strengthening medication knowledge throughout the organization and contributing to the development of medication management and medication use. Part of the work involves education and knowledge dissemination in the operations through, for example, participation in physician meetings, management group meetings, continuing education days, and more. With your pharmaceutical expertise, you contribute to collaboration with strategists, chief physicians, chief nurses, medication-responsible physicians, and other relevant persons. Development and implementation of local instructions on medication-related matters to increase medication safety and patient safety are also part of the role. QUALIFICATIONS We welcome you who are a pharmacy technician, pharmacist, or hold a pharmacy degree with authorization to work within Swedish healthcare. Swedish language proficiency at least corresponding to level C1 according to the Council of Europe's level scale is a requirement. In addition, you have good experience working with medication matters in clinical practice in region-operated activities. Legal knowledge regarding the regulations governing medication handling within healthcare is advantageous for the position. If you have experience in project management or investigative work, that is also considered meritorious. Good English language skills are also desirable. As a person, you are responsible and driven with clear goals and results focus. You are a good communicator who conveys your expertise in a pedagogical manner. Furthermore, you have an easy time building trust and good collaborations with different professional groups at different levels. The department is responsible for appropriation-financed care operations within near care. This includes, among other things, mobile teams and specialized palliative care, outpatient activities within near care, emergency care within near care, 1177 by phone, and certain region-wide functions. Additionally, the department is responsible for developing and monitoring the transition to near care in Region Skåne. The number of employees is estimated to be around 1,000.
