Welcome to us Healthcare Team 9 is part of the healthcare function in Jönköping Municipality, consisting of 13 multiprofessional healthcare teams and one home care team. We have specialist focuses in psychiatry, palliative care, home rehabilitation, dementia, and disability care. Together we are approximately 460 dedicated employees. Our service area covers Barnarp with surrounding countryside, Råslätt, Kettilstorp, Tokarp, Ljungarum, and Gräshagen. We are responsible for healthcare services in four senior living facilities: Havsörnens äldreboende, Junegårdens äldreboende, Kärrhökens äldreboende, and Ädelstenens äldreboende, as well as group housing Kärrhöksgatan. The work team is based at the team office in Råslätt and consists of occupational therapists, physiotherapists, nurses/specialist nurses, nursing assistants, and a unit administrator. Your new job As a pharmacist with us, you'll have a responsible and varied role with a focus on quality and safety in medication management. You'll be an important part of the healthcare function and will be responsible for: educating care staff in medication management ahead of delegation participating with the team in patient admission and medication takeovers maintaining medication storage routines conducting medication procedure reviews for nurses playing an active role in implementing and managing medication reminders The role involves independent work combined with close collaboration in multiprofessional teams consisting of nursing assistants, nurses, occupational therapists, and physiotherapists. Coordination also occurs with external partners such as physicians, health centers, and hospitals, and when needed with family members. Together we create conditions for continuity, efficiency, quality, and high patient safety, so that each individual receives coordinated care and support. Your expertise You are a licensed pharmacist or licensed pharmacy technician. In addition, you have at least two years of professional experience from work at a pharmacy, in clinical pharmacy, and/or in municipal healthcare. Experience from care and support work is advantageous, for example work as a nursing assistant. You have good knowledge of Swedish, both spoken and written, and are comfortable with computers. Since travel occurs in this position, a Category B driver's license is required. We are looking for someone who is self-directed, organized, and flexible in their work approach.
