Storvik Din Hälsocentral currently has approximately 25 employees and provides primary care with physician services, district nurse services, nursing assistant services, child health clinic at family center, psychosocial team, occupational therapy, health professionals, physical therapy, and home visits. We offer specialist clinics for diabetes and hypertension, among others. All employees are highly engaged and participate in developing our operations. Good atmosphere and excellent collaboration between all professional categories characterize our workplace. We currently have around 6,000 registered patients and this number is growing. We are centrally located in Storvik with excellent public transportation by bus and train—only 25 minutes from Gävle and 37 minutes from Falun.

DUTIES
At Storvik Din Hälsocentral, you will work closely with other professionals and meet patients of all ages with different needs. We have specialist clinics for diabetes and hypertension. Here you will work in a varied and stimulating environment with focus on patient-centered care and preventive work.
As a district nurse/nurse, you will work with, among other things:
• scheduled clinic visits
• telephone advice
• primary care acute patients
• digital care meetings through Min Vård Gävleborg.
The position includes some service on the family medicine on-call team in Gävle.

QUALIFICATIONS
We are looking for someone with a pronounced ability to collaborate and build good relationships within the work team. You are attentive and communicative while being independent and taking responsibility for your tasks with great care. Your work style is characterized by flexibility, which allows you to adapt smoothly to new situations and changed circumstances. Furthermore, you also see the connections between different parts of the operation and always act with long-term goals in mind.
In your role as district nurse/nurse, you should:
• be a licensed nurse
• hold a B-class driver's license.
For this position, it is advantageous if you are a district nurse or have experience working in primary care.
