Your responsibilities include investigating, assessing, and making decisions regarding services under the Social Services Act for elderly care and to some extent mental health services.
You will be part of a team of case managers who are skilled at supporting and helping one another. You collaborate with individuals, family members, colleagues, service providers, and other professionals and authorities. An important part of the work is cooperating with service providers in elderly care and mental health services, as well as with external parties such as Region Skåne.
Competencies
We are looking for someone with a degree in social work or equivalent education that the employer deems comparable. We welcome experience in case management within elderly care and mental health services. You have knowledge of current legislation, and it is a merit if you have worked with the Lifecare business system.
To thrive in this role, you need to be organized and collaborative. The work also requires you to be solution-focused, independent, and flexible. You have strong communication skills in both speech and writing. You communicate clearly and ensure information reaches its intended recipients. A significant part of the investigation work involves meeting people in varying situations. It is therefore important that you are responsive and attentive in your interactions with individuals.
A driver's license is required.

Permanent full-time position. Start date by agreement.
