We are now seeking an engaged support and needs assessment officer for Individual and Family Care Services. With us, you will be part of an operation where collaboration, development, and quality are in focus. You will work closely with colleagues in social services and other collaborative partners to create good conditions for the municipality's residents.

Your tasks
As a support and needs assessment officer, you are responsible for investigating, assessing, and making decisions according to the Social Services Act (SoL) and the Act on Support and Services for Certain Persons with Disabilities (LSS). You work with the individual's needs in focus and ensure that appropriate support and measures are provided based on current circumstances.
The work involves handling cases within SoL and LSS, contact with clients, relatives, and other professionals where collaboration is an important part of your duties. You follow up on decided measures and document in accordance with applicable guidelines and legal requirements.
You have the opportunity to plan and structure your work independently, which requires you to be responsible, flexible, and have good prioritization skills.

Requirements
Degree in Social Work or other relevant higher education
Experience in social services authority is meritorious
Class B driver's license
Good knowledge of Swedish, both spoken and written
Good computer skills and experience working in digital systems
Meritorious
Experience handling cases according to SoL and LSS
Experience working in Individual and Family Care Services
Experience with the Lifecare system
Knowledge of relevant legislation and authority
Experience collaborating with other authorities and organizations
