About the job Do you want to be part of a team that helps people build independent lives? The Änggården team is an apartment residence in beautiful Änggården with a central location and good transportation connections. The residence consists of 70 apartments in blocks, and we are expanding our operations to also include integrated apartments. The residence is drug-free and specializes in psychiatric care. Residents are placed here by referral from social services. The purpose of the placement is for the resident to strengthen their housing ability according to the rental law, so they can eventually manage their own apartment. We work through contact management and provide support in accordance with the assignment carried out through the implementation plan. Different support levels guide how much support each person needs. We assist with managing their apartment, provide motivation, help with healthcare contacts, pay bills on time, search for apartments, and interact with authorities and healthcare providers. We have good cooperation with Sahlgrenska/Rågården, which also places patients here according to LRV. As a treatment assistant, you will manage your own cases, conduct home visits, and support the person in their apartment. Be alert to early signs of decline in well-being and support them to seek healthcare contact. By monitoring implementation plans, highlight any needs that need to be raised to the placing case manager for future planning. About you The basic requirement for this position is that you have at least a 2-year YH education as a treatment assistant, social worker, or other education that the employer deems equivalent. Since the role includes driving, you must also have a B driving license and feel comfortable driving in city traffic. You need to have good knowledge of Swedish language in speech and writing, as the work involves extensive contact with residents, colleagues, and authorities, as well as written documentation. We consider it an advantage if you have education and work experience in meeting people with psychotic disorders, anxiety, and PTSD. If you have training in LAB, MI, ÅP, and ESL, that is also an advantage. It is important that you are a team player, help and support colleagues who may need relief during periods when a resident is struggling. That you are flexible and adapt your approach and can reprioritize your day if needed. That you understand the assignment and work toward good relationships with our placing case managers, healthcare providers, administrators, etc. That you are curious about those we work for and interpret charitably. That you are responsive and helpful in your interactions. That you are stable and secure, calm and confident in demanding and difficult situations.
