Hedesunda Färnebo Din Health Center is located 3.7 miles south of Gävle. The catchment area covers Hedesunda and large parts of Österfärnebo and Gysinge. The population base in the area is approximately 3,900 with 3,350 actively registered. Hedesunda Färnebo Din Health Center is part of the primary care collaboration area Gävle, which consists of eight publicly operated health centers. We provide primary care including physician services, respiratory services, home visits, child health clinics, district nurse services, psychosocial team, occupational therapy, and physiotherapy. JOB RESPONSIBILITIES
Are you an occupational therapist who wants to work with us to support our patients' opportunities for activity and health? The position is offered at 100 percent. The need at Hedesunda Färnebo Din Health Center is 50 percent, and the remaining 50 percent involves working as part of the team in the primary care collaboration area Gävle.
As an occupational therapist with us, you will work with, among other things:
• assessment of activity ability in consultation with the patient and rehabilitation needs
• assessments, prescription of assistive devices, fitting, follow-up according to the prescription process
• memory assessments as a complement to the district nurse's and physician's assessment/investigation
• formulate statements in matters concerning home adaptation and applications for personal assistance
• monitoring of assistive device costs and rental responsibility
• patient education and counseling
• individual treatment to a high degree in collaboration with other professionals
• assessments and occupational therapy interventions in the patient's home.

QUALIFICATIONS
We are looking for a flexible and engaged colleague who enjoys being part of the development at the health center and within primary care. You are responsible and solution-focused with good collaboration skills, as we have interdisciplinary cooperation.
In the role of occupational therapist you should:
• be a licensed occupational therapist
• hold a valid B driver's license.
For this position, it is meritorious if you have experience working in primary care and/or home health care.
