With us, you will have independent and developmental work where you, together with a multidisciplinary team, work to create safe and secure hospital discharges as well as follow up interventions during short-term stays. We are a team that works very well together and where we put the team before ourselves. We help each other with our cases to create a comprehensive view of the individual. The team consists of 10 social workers, 5 nurses, 3 occupational therapists, 3 physiotherapists, and 1 assistant. Together we contribute different experiences and competencies that enrich our operations.

Job Responsibilities
As a physiotherapist in the care planning team, you are responsible for safe and secure discharge of patients from inpatient care as well as follow-up during short-term stays. You work from an individual-centered approach with a focus on creating conditions for the individual to live as independently, participatory, and meaningfully as possible.
Together with your colleagues, you plan for hospital discharge through digital planning, personal meetings, or telephone contact. The work also involves making assessments and handoff reports in accordance with the Health and Medical Care Act (HSL) to other operations in connection with hospital discharge and short-term residential care.
You collaborate daily with several different professional groups both internally and externally. An important part of the work is creating good relationships with next of kin as well as working closely with social workers, occupational therapists, and physiotherapists to provide users and next of kin with security and good service.

Qualifications
Requirements for the position:
- Licensed physiotherapist (180 credits)
- Previous experience as a physiotherapist in municipal operations and/or in inpatient care/primary care
- Previous experience with the SVU process
- Very good knowledge of Swedish, both spoken and written
- Good understanding of applicable legislation, established goals, and guidelines
- Good computer skills

About the Workplace
The Authority Division in the Health, Care and Social Services Administration is organized into three units, one of which is responsible for collaboration and planning for discharge from inpatient care as well as follow-up of short-term care decisions. In everyday language, we call ourselves SVU, an abbreviation for collaboration in discharge from closed health and medical care. The unit consists of approximately 80 employees divided into three sections with multidisciplinary teams including social workers, nurses, occupational therapists, physiotherapists/physical therapists, and assistants. The professions in the team make decisions based on their assignments regarding the need for assistance under the Social Services Act (SoL) and interventions under the Health and Medical Care Act (HSL).
