We have not recruited case handlers within the Elderly Unit for several years, which we see as a testament to a stable workplace with good satisfaction. The position is now vacant as an appreciated colleague moves on to a new assignment within the same administration. The Elderly Unit is part of the Social Services Department's authority division. Within the Elderly Unit, ten case handlers currently work together with a fee handler, a handler for transportation services and national transportation services, and a housing coordinator. The operation is led by a group manager and unit manager who together are responsible for developing and supporting the operation. Job Responsibilities: As a case handler in elderly care, you are responsible for case handling, investigation, decision-making, and follow-up according to the Social Services Act (SoL) for persons aged 65 and older as well as for persons under 65 with dementia. Decisions are made according to current delegation regulations, where case handlers have extensive decision-making authority within the operation's area. You meet individuals with different needs and life situations and conduct investigation meetings in the individual's home, at the hospital, or in our offices. To create more time for professional meetings and reduce administration, we explore and develop new digital work methods, including AI-based solutions, such as support in documentation work. The work also involves close cooperation with relatives, service providers, healthcare providers, and other collaborating partners. You independently manage your cases from application to decision and follow-up, and actively contribute to the operation's development and improvement work. At the same time, collaboration and knowledge exchange are important parts of our working methods. We have regular case discussions and case consultations where we together discuss complex issues, create consensus, and contribute to a learning organization. Qualifications: We are looking for you who have: A degree in social work or other higher education that the employer deems equivalent. At least two years of continuous experience with authority work involving independent case handling, investigation, decision-making, and follow-up according to the Social Services Act (SoL) and/or the Act on Support and Service for Certain Persons with Disabilities (LSS). Experience working with evidence-based investigation methodology, such as IBIC (Individual Needs in Focus) or equivalent methodological support. Very good ability to express yourself in Swedish, both orally and in writing. Valid Class B driving license. Meritorious: Experience in handling and investigation according to the Transportation Services and National Transportation Services Act. Experience in municipal operations and politically governed organizations. Experience with the Treserva case management and documentation system.
