Care Coordinator for Malmökraften

Malmökraften is a collaboration between the Swedish Public Employment Service, the Social Insurance Agency, the City of Malmö, and Primary Care in Region Skåne. Malmökraften is now integrated into the Swedish Public Employment Service's regular operations with financing for certain services from Finsam 2025-2027. The project's aim is to get Malmö residents who are far from the labor market into employment, work, or studies. Success factors include team-based work where employees' different professions and organizations work together around Malmö residents who need support. The twelve multidisciplinary teams include employment officers, labor market secretaries, social secretaries supported by study and career counselors, social insurance advisors, and two care coordinators who provide consultative support to the teams. Malmökraften is governed by a decision-making group with representatives from the participating organizations. JOB DUTIES As a care coordinator, you support the participant/Malmö resident in the process toward work or studies through, among other things, mapping conversations to clarify the need for interventions. You will be the link to Primary Care to clarify any medical needs and provide consultative support to half of the twelve multidisciplinary teams. As a care coordinator, you have regular meetings with the teams to identify participants who need support contact with a care coordinator in order to support the participant in contacts with healthcare. Follow-ups of interventions are also part of your role. You will work extensively in the PMO journal system where you interpret various medical statements to use as a basis in coordinated contacts with healthcare. Collaboration occurs with, among others, rehabilitation coordinators at health centers, and you also prepare coordinated individual plans (SIP). You will work with both public and private health centers, so you also need to be comfortable informing about your professional role and the purpose of interventions for participants at different providers. The position is currently physically located in the Swedish Public Employment Service's fine premises opposite City Hall in Malmö. QUALIFICATIONS We are looking for you who are an authorized healthcare social worker or authorized occupational therapist with authorization to practice within Swedish healthcare. If you have another authorized profession within healthcare and authorization to practice within Swedish healthcare, this may also be assessed as meeting the requirements for the position. Swedish language skills at least level C1 according to the Council of Europe's level scale is a requirement. You have experience working in primary care with good knowledge of PMO and interpretation of medical statements and are well-versed in primary care work methods. It is meritorious if you have experience from work where coordination and collaboration with other providers were included, such as work as a rehabilitation coordinator (Reko). If you have knowledge of labor market issues and the regulations of the various organizations involved, this is also meritorious. You also need good knowledge of Region Skåne's healthcare services.
