Center for Movement Organs Umeå is responsible for Orthopedics in the northern healthcare region, which includes the four northernmost counties. Center for Movement Organs is divided into the sections Arthroplasty and Tumor, Factum, Trauma and Spine, all of which perform thousands of patient visits, treatments and surgical procedures annually. Center for Movement Organs, Umeå is a University Healthcare Unit (USVE) where research and student supervision are a natural part of daily clinical work. Within Center for Movement Organs Umeå there are outpatient clinics, inpatient wards, day surgery unit, administrative unit and orthopedic technical center. Our care is characterized by a holistic approach where many different professional categories are engaged in the patient's care process and collaborate for the best results with the vision of keeping Norrland on its feet and in motion. The Orthopedic ward works closely with Umeå University through our newly opened clinical education ward. We are now seeking a Discharge Coordinator who participates in the discharge process for the Orthopedic ward's inpatient patients 1 position(s). JOB RESPONSIBILITIES As a discharge coordinator with us, you contribute to taking good care of others and helping people feel secure. In the Orthopedic ward you will have the opportunity to learn a broad range of orthopedics, from planned admissions to acute care including trauma and advanced spine surgery. With us, the discharge coordinator has an important and natural role in the care team where you as a coordinator coordinate planning around the patient before they leave inpatient care. As a discharge coordinator you work closely with the patient, their relatives, the care team and other healthcare providers. We work in fast care chains where mobilization and return to previous life are pursued after surgery. As a discharge coordinator with us, you identify risks, assess needs and coordinate care for a safe discharge home. QUALIFICATIONS We are looking for you as an engaged colleague who contributes with knowledge and joy, who feels responsibility for the patient's wellbeing and the continued development of the operation. You have a licensed healthcare profession that the employer considers relevant to this position, with an interest in orthopedics and acute care. You have extensive experience in the care chain on the inpatient ward. You can handle multiple tasks in parallel and see it as a challenge to work at a varying pace. You are responsive to the patient's needs and enjoy collaborating with people in different professions and actively contribute to high patient safety and a good work environment through your knowledge and dedication.
