Humana Sockerstan is a care home for those in need of somatic care or dementia care. At Humana, you will work from a person-centered approach, where each individual's wishes and needs are in focus. Working Hours and Employment Type: Days, evenings, and weekends. Permanent employment with a six-month probationary period. Number of Positions and Employment Level: 4 positions at 75%. You will daily document and handle any deviations. Together with colleagues you distribute work tasks and communicate with HSL personnel such as registered nurses, rehabilitation staff, and on-call nurses. Examples of Work Tasks: Be a contact person and responsible for your clients' weekly planning and implementation plan Follow up on your clients' weekly planning and implementation plan Receive medication delegation from registered nurses Carry out fun activities Handle documents and deviations Who Are You? We are looking for you who is a team player – a person who is responsible and enjoys working both independently and in a group. You have the ability to see the bigger picture and you are not afraid to take initiative. You are experienced with documentation and work with quality as your guiding principle. Experience with person-centered work is a merit and we believe it is important that you are a relationship builder with good cooperation skills and a positive mindset. We are Looking for You Who: Are a trained nursing assistant or have equivalent work experience Have experience working with people with dementia Possess good knowledge of Swedish in speech and writing It is a Merit if You Have: Experience working in elderly care or home care services Experience in privately run operations A prerequisite for starting employment with us at Humana is that you as an applicant can present a valid ID document and any documents proving your right to work in Sweden.
