We at the Mobile Team are now looking for a new colleague to join our fantastic team! The Mobile Team implements Safe Discharge Home for those who need support after a hospital stay. Work Tasks With us, the detective in you has room to make a real difference for our citizens. You map, identify needs, and work preventatively to enable our users to remain living at home. By motivating and strengthening the individual's own abilities, you solve the puzzle together. With us, you work independently but never alone. We are a small work group that collaborates closely with rehabilitation, nurses, benefits administrators, and home care teams. Together we solve the tasks! We have the entire municipality as our workplace and carry out shorter interventions before regular home care takes over. This means no two days are the same. You meet users throughout the municipality and get great variety in your work. You are expected to take great responsibility and are confident in your competence to make independent assessments. Thorough documentation is an important part of your work. We offer you meaningful work where you have the opportunity to deepen your knowledge in palliative care, dementia, everyday rehabilitative approaches, and transfer techniques. You also have the opportunity to influence your working hours together with your colleagues! Qualifications We are looking for you who are a nursing assistant or have other education or experience that the employer deems sufficient. We would like to see that you have further education in elderly care and/or dementia or have an educational background in psychiatry. Experience working in care and welfare is a merit. To succeed in this position, it is important that you are confident in your role. You are self-motivated, solution-focused, and enjoy driving your own work forward. It is important that you like and enjoy working with people and that you are flexible and open to new ideas and ways of working. The position requires sufficient knowledge of Swedish in speech and writing in order to, among other things, communicate with users, perform care documentation, and receive medication delegation. A valid driver's license, category B, and access to your own car are required. Include a CV and personal letter with your application. Selection and interviews are conducted on an ongoing basis. In the Welfare Department, approximately 2,500 employees work. The department is governed by the Welfare Board based on the department's mission – "Together we facilitate people's opportunity to live a meaningful life" – we work to achieve the municipality's vision: We build best together. In connection with the interview, we will conduct an identity check and verify citizenship. Accepted documents are passport, international ID card, driver's license + identity certificate from the Swedish Tax Agency, or other ID documents that strengthen citizenship. For some positions, a printout from the police's suspicion and criminal record register is required. We would like to see language skills in Finnish and/or one of the Sámi varieties, as Örnsköldsvik Municipality is part of both the Finnish and Sámi administrative areas. In our municipality, a number of positions are classified as security-classified, so security vetting may be conducted.
