The Discharge at Home Team in Norrköping is based at Hantverkaregatan 51 and is a unit with nurses, occupational therapists, physiotherapists, and nursing assistants. The team's mission is to provide support with care and assistance, including rehabilitation, to people with extensive and changing needs that cannot currently be addressed through regular home care services. The need for the Discharge at Home Team's services may arise after a hospital stay, a stay in short-term housing, or in connection with home care matters in the individual's own home. Your tasks Your work as a nursing assistant in the Discharge at Home Team is important and you influence how other people experience their daily life. You provide security and support with your expertise when supporting patients transitioning home after hospital stays. You will be part of a team where the individual's care and support needs are the focus. Together, the team creates a sense of security around the individual with a focus on a rehabilitative approach. As a nursing assistant in the Discharge at Home Team, you work days, evenings, and weekends. Who are you? We are looking for you who are a trained nursing assistant according to the National Board of Health and Welfare's regulations. You have experience with typical nursing assistant tasks. We value a positive attitude, commitment, flexibility, and a sense of service. As a person, you are confident, stable, and responsible. You have the ability to conduct yourself in a way that is adapted to the situation and you structure and manage your work both independently and in collaboration with the team. You are attentive with the ability to put yourself in other people's situations. You adapt easily to changed circumstances and see changes as an opportunity. You master the Swedish language in speech and writing. You have a driver's license and can cycle. We welcome your application!
