About the Company Waxö Omsorg is a small care company that provides support, services, and care to adults with cognitive and neuropsychiatric disabilities and individuals with acquired brain injuries. The needs, wishes, and integrity of each individual are guiding principles for all activities within Waxö Omsorg. A key premise is that care recipients should be offered the support they need to live as independently and fulfilling a life as possible. We strive for the care recipients we are responsible for to achieve a high degree of independence and feel valued and participate in society in the same way as other residents of the municipality. Waxö Omsorg has a collective agreement with parties according to the Care and Social Services Agreement, Swedish Municipal Workers' Union (Kommunal). Waxö Omsorg is a member of the Municipal Companies' Co-organization (KFS). About the Workplace Storgatans Gruppbostad is an LSS residential facility for individuals with extensive support needs. Six care recipients live here who have been granted support services under LSS Section 9, Paragraph 9, and receive support in their daily lives at home and during leisure time. How support is provided is described in each care recipient's implementation plan and is based on their wishes and needs. There are clear job descriptions for staff, and as a new employee you will receive an introduction to working with care recipients as well as documentation guidelines. An introduction program is available and its length is adapted to individual needs. We are a smoke-free workplace. About the Position We have a 100% position with responsibility as a contact person. The schedule includes days, evenings, and weekends. As a contact person, you have special responsibility for several care recipients. Some of the main responsibilities are to: - Offer care recipients the opportunity to live a good life based on applicable legislation and agreements for the organization. - Plan, motivate, and carry out activities based on the care recipient's interests, wishes, and needs to promote participation, independence, social community, and meaningful leisure time. - Contribute to care recipients being offered social, cognitive, and sensory stimulating activities that strengthen quality of life, well-being, and opportunities for development. - Document the care, support, and services the care recipient wishes to receive. - Create and continuously revise an implementation plan together with the care recipient. - Identify and document existing risks and, where possible, eliminate risks in the care recipient's home and leisure environment. - Document consent regarding information sharing and cooperation. Based on consent, establish routines for cooperation with relevant individuals, organizations, and authorities. - Ensure that we meet established goals for the care recipient based on agreements with the municipality. - Ensure that care recipients have the assistive devices they wish for and need, and together with the care recipient find and develop cognitive aids that increase the care recipient's independence. - Ensure that care recipients are offered annual health and dental check-ups and contact with healthcare services when needed. - Ensure that care recipients' apartments are clean and that good hygiene is maintained in the kitchen and bathroom. - Inform the staff team at meetings about how care recipients wish to receive support, what routines apply, and receive information from staff. About You You have knowledge of current legislation required to perform the work (LSS, SoL, HSL). You have knowledge of intellectual disabilities and understanding of how the disability can affect an individual's communication, participation, independence, and support needs. You can adapt your approach, manner, and communication based on the care recipient's individual needs, prerequisites, and cognitive abilities. You have good communication skills to build trusting relationships with care recipients, colleagues, family members, and other professional actors. You should be responsible and reliable as you have a central role in the care recipient's daily life. You should be flexible and able to adapt to different situations and find solutions. You should have good knowledge of how to document in accordance with applicable guidelines and laws. You should be able to demonstrate your knowledge and experience by providing course certificates, employment certificates/service references, and providing two references from previous supervisors/coordinators.
