Description Would you like to be part of developing collaboration between social services and healthcare? The new Social Services Act, the shift toward care closer to home, and an increased need for coordination between authorities are placing new demands on social services' collaboration with healthcare. We are now seeking someone who wants to help develop care coordination for Gothenburg City's four social welfare administrations within individual and family care. As a process manager, you will have a central role in city-wide development work. You will support the social welfare administrations in developing common working methods, processes, and structures that strengthen collaboration with healthcare and contribute to a more equitable and legally sound social service. You will become part of the Quality Unit, a city-wide support function for Gothenburg City's four social welfare administrations within individual and family care. We develop common processes, create governing documents, and support operations in development issues related to the social services process. The unit consists of nine process managers and is led by a unit manager. Work Duties As a process manager, you will have a central role in developing social services' work within care coordination. You will work both strategically and operationally to lead development work, support the four social welfare administrations, and create conditions for more cohesive collaboration with healthcare. Your role includes, among other things: Facilitate work groups and coordinate development initiatives, Support operations in development and collaboration matters, Identify development areas and develop social services' working methods within care coordination, Implement, follow up, and develop common processes, routines, and agreements, Represent the social welfare administrations in networks, work groups, and collaboration forums, Analyze results and develop routines, support documents, reports, and decision-making materials. The assignment includes work with SIP, SAMSA, admission and discharge processes, deviation management, and other common processes within care coordination. The role involves close collaboration with the city's social welfare administrations and dialogue with other relevant collaboration partners. Qualifications To thrive and succeed in this role, you have a degree in social work (socionomexamen) or other education that the employer considers equivalent and relevant for the assignment. You have good knowledge of care coordination and are well versed in work with SIP and SAMSA as well as how collaboration between social services and healthcare functions in practice. We would like you to have experience in authority work within social services and good knowledge of healthcare. It is also advantageous if you have experience leading development work, for example through process management, team leadership, or implementation work. As a person, you are driven and have a genuine interest in development and improvement. You have the ability to shift between a strategic, holistic perspective and operational matters and feel comfortable driving development work together with others. You work independently and in a structured manner, while at the same time you find it easy to build trusting relationships and collaborate with many different actors in a complex organization. Since the role involves dialogue, coordination, and documentation, you have excellent ability to express yourself well in speech and writing.
