As part of the Mobile Primary Care Team, you'll be part of a multidisciplinary team that together creates safe, accessible, and patient-centered care for patients in their homes. Mobile Primary Care Teams exist throughout Scania and are very positively received by both patients and staff. In October 2024, we started the team in Lund with a catchment area covering Lund, Eslöv, Burlöv, Staffanstorp, Kävlinge, Lomma, Höör, and Hörby. We collaborate with SUS Lund, Centralsjukhuset in Kristianstad (CSK), ambulance services, private and public health centers, and other healthcare providers in the area, such as the health and care administration in each municipality. The Close Care and Health Department is being established and will be operational from January 1, 2025. The department is responsible for publicly funded healthcare activities within close care. This includes mobile teams and specialized palliative care, outpatient services in close care, acute care in close care, 1177 telephone services, and certain regional shared functions. Additionally, the department is responsible for developing and monitoring the transition to close care in Region Scania. Mobile Primary Care Teams represent an integrated care model where primary care is the hub in collaboration with other healthcare providers. The team offers safe, clear, and accessible care for patients who cannot come to the health center and who need healthcare provided in their homes instead of in hospitals. The team can assess, plan, prescribe, and deliver interventions. Care is specialized based on each patient's individual needs, and the goal is to provide patients with qualified care both medically and psychosocially. The team is based in Lund and consists of physicians, nurses, a social worker, and a medical secretary. Hours of operation Monday–Sunday, 8 AM–5 PM. 1 position(s). RESPONSIBILITIES Your role involves working in a team where you make home visits to patients in the various municipalities. You lead and actively participate in planning patient care together with other professionals and collaborate with other healthcare providers. When patients may need hospital care, we strive for direct admissions to wards. Participation in the operation's development and improvement work is also part of your responsibilities. Working hours are 8 AM–4:30 PM, Monday–Friday. On-call duty on holidays approximately one shift per month, according to primary care agreements. QUALIFICATIONS We welcome you if you are a licensed physician with authorization to practice within Swedish healthcare and have specialist competence in a relevant area. Swedish language proficiency equivalent to at least level C1 according to the Council of Europe scale is a requirement. A valid B driver's license is also required, as care is largely provided in patients' homes. Experience from similar work is a merit. You are confident in your professional role and keep the patient in focus. You have the ability to see each patient's individual needs and a flexible approach to organizing the work day. Part of the work involves support and conversation, which requires that you are communicative and have the ability to meet people in different stages of life.
