Division Nära, VO Psychiatry has initiated a transition to Nära Vård (Close Care), where we are moving care into patients' homes, closer to where they are. The clinic has therefore developed a specialist psychiatric care team, SPOT, which in June 2025 was decided to be implemented as a permanent operation. About You We are looking for you—a social worker with experience in authority work and collaboration. You have good computer skills and strong proficiency in Microsoft Office, and you communicate well in Swedish, both spoken and written. We consider it a merit if you hold a license as a health and social care coordinator, have experience from psychiatric work, or have knowledge of a Swedish national minority language. We place great emphasis on personal qualities. As a person, you are mature, structured, and self-directed. You are flexible and adapt easily to changing circumstances. Your Work The purpose of SPOT (Specialist Psychiatric Care Team) is to provide more individually tailored care interventions in the home in an outpatient setting, rather than in inpatient care. The goal is to reduce the need for inpatient care time for patients and avoid hospitalization when possible, as well as to strengthen support where ordinary psychiatric outpatient services are not deemed sufficient. Care shall be person-centered and adapted to each patient's individual needs, capabilities, and condition. SPOT's target group consists of patients within the Piteå General Psychiatry catchment area, 18 years of age and older, whose care needs exceed what the psychiatric outpatient services can provide in regular operations, but who are not deemed to require 24-hour inpatient care. SPOT is available every day of the year. Work tasks within the SPOT team may vary but include, for example, supportive conversations, behavioral observations, anxiety management, telephone follow-ups, working in resource teams around patients with complex issues, follow-up of patients from inpatient care, support for patients on leave, family conversations, and collaboration with care partners and municipalities. SPOT works on assignment from physicians in inpatient or outpatient care. As a social worker in SPOT, you are responsible for supporting staff in matters concerning collaboration, informing patients about their available support options, providing family support, and participating in collaboration meetings with our care partners and municipalities.
