Your role is to be a strong partner in collaboration with hospital care and primary care. The planning unit's main duties: • The primary task is, in accordance with SOL, to assess the need for support in connection with hospital discharge and plan for a safe return home. • Collaborate with the region, primary care, and municipal primary care in connection with admissions and discharges based on the individual's needs. • Follow up in the short term. The planning unit follows up on short-term decisions until the person leaves the short-term unit. • Active collaboration with other professionals to achieve a safe return home About You Requirements • Social Work degree Meritorious • Experience in public administration. • Familiarity with admission and discharge procedures from hospital care. • Knowledge of IT services SAMSA and Treserva.
