Kleofáše o.p.s. is looking for a colleague for the position of SOCIAL WORKER Work location: Třeboň Start date: by agreement Job responsibilities: - establish initial contact with individuals interested in hospice care - conduct social assessments - provide expert social counseling - maintain social documentation - provide psychosocial support to clients and their loved ones - work using case management approach - actively collaborate within a multidisciplinary team and regularly participate in its meetings - cooperate with authorities and palliative care providers - participate in the development and updating of social service standards - participate in community planning of social services, etc. What we need from you: - university education and professional qualification in accordance with the law on social services - knowledge of the law on social services no. 108/2006 Sb. including implementing regulation no. 505/2006 Sb. - active driving license - knowledge of computer work - alignment with the organization's mission and value code - pleasant demeanor and good communication skills, empathy, ability to listen, courage to work with difficult topics, organization, reliability, responsibility, kindness, independence, attention to detail, ability to work as a team, desire to continue learning - clean criminal record What we offer: - further professional growth through systematic education in palliative care - work in a friendly, experienced and supportive team - meaningful and diverse work with space for personal development - provision of psychological support and supervision - appropriate financial compensation with growth potential (starting salary for full-time 30,600 Kč, after probation period 33,150 Kč; starting salary can be individually increased based on proven experience and professional expertise) - 5 weeks of vacation, meal allowance of 129 Kč/day - work contract 0.75 to 1.0 full-time If you are interested in the offered position, please send your CV along with a motivational letter by September 7, 2026 by email to: [email protected].
