A SOCIAL WORKER.
Project Objective:
The project provides individualized, comprehensive support for children with behavioral and addiction problems, as well as their families, based on collaboration among specialists from various fields. This enables families to receive necessary assistance in a timely and coordinated manner in one location.
Service delivery period from November 2026 to August 2029.
1. Primary Responsibilities:
Support Service:
Ensures implementation of the Support Plan, its dynamic performance evaluation and monitoring within professional competence, including identification of the child's and, where applicable, the attached person's needs, resources and risks during service delivery. Documents the Support Service process, organizes regular information exchange within the interdisciplinary team and prepares recommendations for the MA interdisciplinary team.
Housing Service: Ensures implementation of the Support Plan, its dynamic performance evaluation and monitoring within professional competence, including identification of the youth's resources and risks during Housing Service delivery. Documents the Housing Service process, organizes regular information exchange within the interdisciplinary team and prepares recommendations for the MA interprofessional team.
SA Service:
Reviews information from the MA service and conducts case management for the SA service. Ensures information provision and feedback reception from the child, attached person, family, cooperation partners and contact persons in Support Plan implementation. Organizes team meetings for Support Plan implementation and monitors Support Plan execution. Ensures interim evaluation of the Support Plan after 6 months and organizes final progress assessment of the child and attached person upon SA service completion.
Provides consultation to the attached person at least once a month (or more frequently, if necessary) and informs about current matters and further actions in Support Plan implementation. Upon completion of SA service, prepares recommendations for the child and attached person, compiles recommendations from all SA service specialists, develops a summary report and submits it to the SA service manager.
Participates and ensures cooperation with other SA service specialists and cooperation partners, communicates with the MA service, attends supervision (1× per month), participates in team meetings (1× per month or more frequently, if necessary), engages in knowledge development activities. When necessary, leads attached person skills development groups and adheres to ethics, data protection and confidentiality principles.
2. Requirements:
First-cycle higher education in social work or charitable social work.
Experience in the past five years working with families with children (minimum 24 months of cumulative work experience)
Competence in case management, social situation analysis and social rehabilitation planning.
Ability to work in an interprofessional environment, adhering to a unified methodological framework.
Ability to act professionally in risk situations, cooperating with other involved institutions within professional competence.
Project Implementation Location:
Daugavpils State City Municipality Institution "Social Service" in cooperation with the Foundation "Latvian Children's Support Fund" and the Association "Resilience Center".
Remuneration: 11.09 euro/h.
