This means you independently assess, plan, and organize medical and nursing interventions. You are responsible for supervising, training, and delegating care personnel. You work in a patient-centered manner, involving patients in decisions and collaborating closely with family and the team around the patient. You regularly collaborate with primary care from a patient focus perspective. You will also work with multiple actors according to the health and medical care agreement, which includes the region, municipality, and primary care, to meet individual care needs. Qualifications We are looking for you who are a licensed nurse with at least two years of professional experience. In addition, we would like you to have: Experience collaborating with various actors in health and care Good digital skills and experience with documentation Knowledge of applicable legislation, goals, and guidelines in the health and medical care field Very good knowledge of Swedish, both spoken and written Class B driver's license Able to cycle Meritorious: Experience as a nurse working with people with dementia Experience working in municipal health and medical care Experience working in the Procapita documentation system You are confident in your professional role and take responsibility for independently planning, organizing, and prioritizing your work. In collaboration with others, you are responsive and communicative and contribute to good cooperation around the patient. You communicate clearly and ensure that information and expectations reach those involved. The role includes supervising and guiding care personnel. This requires you to be able to create engagement and participation and contribute to work being driven toward common goals.
