We are now looking for two committed and confident nurses who want to become an important part of our operations in a rural area. One position is placed in home care in Hamrånge, where you work close to patients in their home environment. The other position is a combined role where you work both in home care and at Solberga care and housing facility, with the opportunity to vary your work between these operations. Within home care, you meet patients where they feel most secure in their own homes. Your contribution is important for their health, safety, and quality of life. As a nurse with us, you have an independent and central role with great variety in your work tasks. You are responsible for both medical interventions and close support to patients and their relatives. The work is conducted in close collaboration with doctors at the patient's health center as well as with home services, creating safe and coordinated care for each individual. Your work tasks include: • Perform healthcare interventions in the patient's home and at the facility. • Be responsible for nursing care and make medical assessments. • Work in a team with physiotherapists, occupational therapists, and doctors. • Guide and delegate to nursing staff and students. • Document in Treserva and other operational systems. • Work preventatively and health-promoting. • We look forward to welcoming you to Hamrånge Home Care and our team! The position is primarily based on daytime work, but the schedule includes evening and weekend shifts. You will then work in our consulting operations, which include the municipality's care and housing facilities, home care, services, and group homes within LSS and social psychiatry. Interviews may be conducted on a rolling basis during the application period. QUALIFICATIONS You are a confident and organized nurse with genuine commitment to patient-centered work. In this role, you have a central and significant function for our registered patients within home care. You work independently with great responsibility while also collaborating closely with other professionals in a team to create safe and coordinated care. Requirements for the position: • Licensed nurse, preferably with several years of experience. • Driver's license and driving experience with a car. It is desirable if you have experience in municipal health and care services and knowledge of the following systems: Treserva, TES, Lifecare, NPÖ, Pascal. You have good collaboration skills and treat patients and colleagues with empathy and respect. You are unpretentious and contribute to a positive team climate. You work in an organized manner, prioritize effectively, and use resources in the best way. You take initiative, are confident in your actions, and distinguish between facts and persons.
