WORK TASKS As a nurse/district nurse, you will work with healthcare in the home, both in ordinary housing and in special accommodations. You are responsible for planning and implementing care measures based on the individual's needs and the patient's best interests while collaborating with other professions. The role includes, among other things: • Plan, assess, implement, and evaluate nursing care. • Assess patients' health status through both patients' subjective experiences and objective data. • Work preventively by identifying, noting, and communicating risks, both for the individual and in the organization, and actively contribute to systematic patient safety work. • Note and act appropriately on changed situations and conditions based on your nursing responsibility. • Perform advanced care. • Provide support and advice to other professionals around the patient. • Maintain good contact with relatives and create security for the patient. • Perform documentation and other administration related to patient care. • Collaborate with other professions As on duty in the evenings and weekends, the nurse/district nurse has a leadership function for nursing staff working in ordinary and special accommodations. QUALIFICATIONS • Licensed nurse, preferably with specialist training as a district nurse but not a requirement. • Experience from municipal healthcare is meritorious but not a requirement. • You are independent, responsible, and have good communication skills. Good Swedish in speech and writing is of great importance. • You have a good manner and ability to create trusting relationships with patients, relatives, and colleagues. • You have a positive attitude and an interest in change and development within healthcare. • B driver's license is required. You are an engaged team player who wants to be part of developing municipal healthcare in Gnosjö municipality together with your colleagues. With your ability to plan, organize, and prioritize, you contribute to safe and efficient care. You are responsive and trustworthy in meetings with patients.
