About the Workplace We are a small, family-oriented health center located within walking distance of Upplands Väsby commuter train station. We currently have approximately 5,000 registered patients. Our home care service covers just over 70 patients. Our home care team consists of two registered nurses and two assistant nurses. We conduct home visits by car or e-bike/bicycle. Responsibilities You will be responsible for your own patients together with the assistant nurse on your team, and you will conduct patient admissions and discharges. Conduct/schedule care planning meetings (SIP) as needed, maintain contact with family members, other units, and the municipality. You will also make daily home visits. Other typical home care nursing duties. Qualifications We are looking for a licensed registered nurse or district nurse interested in working in home care. Several years of experience in primary care/home care is a merit. Valid driver's license (Category B) is required. Good Swedish language skills, both spoken and written. Personal Qualities You are a collaborative person with strong communication skills. You have good organizational abilities and can plan long-term. You are flexible, attentive, and thorough. You can take initiative and are confident in your profession. You can work independently while also functioning well as part of a team. Additional Information Interviews are conducted on a rolling basis and the position may be filled before the application deadline.
