Stadsvikens Health Center is one of the largest in the region, with approximately 15,500 registered patients. We are approximately 60 employees working forward-thinking in close collaboration to develop our work methods for the benefit of our patients and to create a good work environment. We are looking for We are seeking a licensed registered nurse who can become a care coordinator for our psychosocial team. It is advantageous if you have training as a care coordinator; otherwise, there is an opportunity to complete this training on the job. Great emphasis is placed on personal qualities. As a person, you are confident and can work independently. You lead the team with commitment and have a positive attitude. You have a forward-thinking approach and enjoy working with development. Experience from psychiatry is a plus. What you will be working with The team consists of CBT therapists, and when needed, occupational therapists and physiotherapists are available. The team has weekly team rounds where you as care coordinator play an important role. You lead and distribute incoming cases in close collaboration with the rest of the team. You are the patient's first point of contact and conduct assessments, medication follow-ups, and provide supportive conversations to some extent. In addition, you have phone hours and communicate via chat. Each week the team has rounds together with a doctor at the health center, and every third week with a doctor from psychiatry.
