About the Facility
Rättvik Health Center has approximately 10,300 registered patients and we are about 55 employees. Multiple different professions work here and we have close collaboration with one another. We have well-functioning specialist nurse clinics in the areas of Diabetes, Hypertension, Substance Dependency, Dementia, nursing services, care coordination, and coordination with our care partners. This fall, we will begin developing a new way of working within primary care.
Both nationally and locally, there is a need to develop primary care's work methods to meet increasing care needs, improve continuity, and create a sustainable work environment. By creating conditions for increased continuity where the patient belongs to a cross-professional team with shared responsibility.
This way of working strengthens the opportunity to offer consistent care contact, increase patient safety, and create security for both patients and employees. Continuity is not built solely on an individual employee but on a team that together has knowledge of the patient's needs and can ensure cohesive care.
Job Duties
We have had the opportunity to strengthen our clinic and are seeking an engaged nurse.
You will work with competent and pleasant colleagues where our patients are always in focus and where you as staff will thrive and have the opportunity to contribute to a good work environment and develop within your profession. As a nurse with us, you will have a varied and meaningful workday with both clinic work and acute assessments. You work independently but closely with colleagues from various professions.
Main duties:
Clinic work, telephone advice, and 1177 digital advice.
Assessment and handling of acute patient matters
Follow-up of patients with chronic diseases
Team work and coordination of patient care
Participation in improvement and development work
Opportunity to work with specialist clinics based on the facility's needs and interest.
Placement is made by agreement. Selection is ongoing. Good introduction is provided.
Qualifications
Requirements:
Registered nurse license.
Driver's license category B
Meriting factors:
Specialist education as district nurse.
Experience from nursing profession.
Experience from primary care, acute care, or home health care.
Interest and/or knowledge of coordination with the municipality's nurses, benefits officers, and other actors.
Personal Qualities:
You thrive in a role with varied duties and can handle both planned and acute situations. You have a genuine interest in coordination and close collaboration with the municipality's care actors. We are seeking someone who is confident in their professional role, takes responsibility, and has a good ability to work independently as well as in a team.
